What It Is

Crohn's disease is a chronic autoimmune inflammatory bowel disease. The immune system attacks the wall of the digestive tract, producing inflammation that can appear anywhere from the mouth to the anus — every inch of the GI tract is potential territory. That reach is what makes Crohn's one of the most complex and variable conditions in gastroenterology.

Most people picture Crohn's as a colon disease. That is where the confusion with ulcerative colitis starts. The primary territory of Crohn's is actually the terminal ileum — the very end of the small intestine — and the ileocecal junction where the small intestine meets the large intestine. That location puts Crohn's ileitis squarely in the right lower quadrant of the abdomen, which is exactly where appendicitis pain lives. The misdiagnosis rate between the two is significant and documented. If you have had a appendectomy that came back with a normal or near-normal appendix and your GI symptoms continued afterward, Crohn's belongs in the conversation.

The inflammation in Crohn's is transmural — it goes through the full thickness of the gut wall. Not just the surface lining. All the way through. That depth is what separates Crohn's from ulcerative colitis anatomically, and it is what drives the most serious complications: strictures that narrow the bowel, fistulas that tunnel through the wall into surrounding tissue, and abscesses that form when those tunnels become infected.

Surgery does not cure Crohn's. The disease can recur in remaining bowel after resection. That is a fundamental distinction from ulcerative colitis, where colectomy cures the disease. Anyone discussing surgical options for Crohn's needs to understand this going in.

What It Feels Like

The symptom picture in Crohn's is heavily influenced by where in the GI tract the disease is active. Two people with Crohn's can have dramatically different experiences depending on location, extent, and depth of involvement.

The core symptoms:

  • Right lower quadrant abdominal pain — the most common presentation when the terminal ileum is involved. Cramping, often after eating, often severe enough to be mistaken for appendicitis in acute presentations.

  • Diarrhea — chronic, often without blood when disease is limited to the small intestine. Bloody diarrhea when the colon is involved.

  • Fatigue — significant and chronic. Combination of systemic inflammation, malnutrition, and anemia.

  • Weight loss and malnutrition — the small intestine is where nutrients are absorbed. When Crohn's damages or inflames the small intestine, absorption fails. Malnutrition in Crohn's is real, serious, and frequently underaddressed.

  • Fever — during active flares, particularly when abscess or fistula is involved.

  • Nausea — especially with upper GI involvement or strictures slowing transit.

  • Perianal disease — fistulas, fissures, and abscesses in the perianal area. Present in a significant subset of Crohn's patients and often the first visible sign that something is wrong.

When Crohn's involves the terminal ileum specifically, B12 malabsorption is a direct consequence. The terminal ileum is the only place in the GI tract where B12 is absorbed. Inflammation or surgical removal of that segment means B12 cannot be absorbed through normal digestion regardless of dietary intake. B12 deficiency in that context produces neurological symptoms — numbness, tingling, cognitive changes — that can be mistaken for unrelated conditions. If terminal ileum involvement is documented or suspected, B12 status needs to be on the radar.

Crohn's Disease vs Ulcerative Colitis — Knowing the Difference

This comparison appears on both the Crohn's Disease page and the Colitis & Ulcerative Colitis page. If you are working through the differential between the two, the anatomy is where the distinction lives.

Where it lives. Crohn's disease can appear anywhere from mouth to anus. Its primary territory is the terminal ileum and the ileocecal junction — the very end of the small intestine where it meets the large intestine. UC lives strictly in the large intestine and rectum. That's the boundary. It does not cross it.

The pattern. Crohn's produces skip lesions — patches of inflamed tissue separated by stretches of normal tissue. UC produces continuous inflammation with no gaps, no skip pattern, no normal tissue between inflamed sections. One unbroken zone from the rectum upward.

How deep it goes. Crohn's is transmural — it goes through the full thickness of the gut wall. UC is mucosal — it stays in the innermost lining only. That depth difference is what drives the fistula and abscess complications in Crohn's. UC does not penetrate deep enough to produce those.

Bloody stool. In Crohn's, bloody stool depends heavily on where the disease is located. In UC it is a hallmark — nearly universal in active disease — because the rectum and lower colon are always involved and the mucosal surface is ulcerated.

B12 absorption. The terminal ileum is where B12 is absorbed. When Crohn's involves the terminal ileum, B12 malabsorption follows. UC does not typically affect B12 absorption because the small intestine is not involved.

Malnutrition risk. Crohn's carries significant malnutrition risk, especially when the small intestine is involved. The small intestine is where nutrients are absorbed. The large intestine primarily absorbs water. UC affects the large intestine, so malnutrition risk is considerably lower.

What surgery can do. This is the most important distinction. Surgery does not cure Crohn's — disease can recur in the remaining bowel. Colectomy cures UC. Remove the colon, remove the target tissue, end the disease. That surgical outcome difference is real and it matters when treatment conversations reach that point.

The Complications — What Transmural Means in Practice

When inflammation goes through the full thickness of the gut wall, it does not stop at the outer surface. It keeps going into surrounding tissue. That is how Crohn's produces its most serious complications.

Strictures form when chronic transmural inflammation heals with scar tissue that narrows the bowel. A strictured segment cannot pass stool normally. Partial obstruction produces cramping, bloating, and nausea after eating. Complete obstruction is a surgical emergency.

Fistulas form when transmural inflammation tunnels through the gut wall and creates an abnormal connection between two structures that should not be connected. Fistulas can form between loops of bowel, between bowel and bladder, between bowel and skin, or between bowel and vagina. Each type produces its own distinct and serious symptom picture.

Abscesses form when a fistula tract becomes infected and walls off into a pocket of pus. Abscesses produce fever, localized pain, and systemic illness. They require drainage — sometimes surgical, sometimes percutaneous — before underlying fistula disease can be addressed.

Perianal disease — fistulas and abscesses in the tissue surrounding the anus — affects a significant subset of Crohn's patients and is sometimes the presenting complaint before any abdominal symptoms develop. Perianal Crohn's is not hemorrhoids. It is not a simple fissure. It requires gastroenterology and often colorectal surgery involvement.

Crohn's and Appendicitis — The Diagnostic Pattern Worth Knowing

Right lower quadrant pain, fever, and elevated white blood cell count in Crohn's ileitis is clinically indistinguishable from appendicitis on initial presentation. The physical exam findings overlap. The lab findings overlap. The imaging findings can overlap.

People with undiagnosed Crohn's have had appendectomies because the acute presentation looked exactly like appendicitis. When the appendix comes back normal or near-normal on pathology and GI symptoms continue afterward, Crohn's belongs in the differential conversation — not as a certainty, but as a question that deserves to be asked and properly worked up.

How It's Diagnosed

Colonoscopy with terminal ileum intubation and biopsy is the foundational diagnostic tool. Standard colonoscopy ends at the ileocecal valve. Terminal ileum intubation goes one step further into the small intestine — essential for Crohn's evaluation because the terminal ileum is the primary disease territory. Ask for this by name. Do not assume it will be done automatically.

CT enterography or MR enterography — not standard abdominal CT. Standard CT visualizes the outside of the bowel wall adequately but does not give the detail needed to evaluate the full thickness of the small intestine wall or identify early Crohn's changes. CT enterography and MR enterography use oral contrast to distend the small bowel and provide cross-sectional imaging of the full wall thickness. MR enterography avoids radiation and is preferred for younger patients and ongoing monitoring. Ask for enterography specifically, not a standard abdominal CT.

Fecal calprotectin is a stool test measuring intestinal inflammation. It does not diagnose Crohn's specifically, but a significantly elevated result in the right clinical picture points toward inflammatory bowel disease rather than IBS. Useful for initial workup and for monitoring disease activity.

Capsule endoscopy — a swallowable camera that images the small intestine from the inside — can visualize small intestine segments that colonoscopy and standard imaging cannot reach. Used when other testing is inconclusive and small bowel Crohn's is still suspected. Note: capsule endoscopy is contraindicated if strictures are suspected — the capsule can get stuck.

Standard blood work — CRP, ESR, CBC with differential — reflects systemic inflammation, anemia, and white blood cell activity. Not diagnostic alone but part of building the full picture.

B12 level — should be checked when terminal ileum involvement is documented or suspected.

Ask for by name:

  • Colonoscopy with terminal ileum intubation and biopsies

  • CT enterography or MR enterography — not standard abdominal CT

  • Fecal calprotectin

  • CRP, ESR, CBC with differential

  • B12 level if terminal ileum is involved

  • Iron studies if anemia is present

Where Crohn's Lives in the Transit Timeline

The gut transit time map helps sort GI symptoms by location and timing.

  • Stomach emptying: 2–4 hours

  • Small intestine transit: 4–6 hours

  • Large intestine transit: 10–59 hours, averaging around 36 hours

  • Total mouth to toilet: 24–72 hours, averaging approximately 40 hours

Crohn's primary territory — the terminal ileum — sits at the end of the small intestine transit window. Pain and cramping that develops in the hours after eating, localized to the right lower quadrant, fits that timing. Symptoms that appear within one to three hours of eating and involve upper abdominal bloating are more consistent with SIBO territory. Symptoms that build over 24 to 48 hours and take days to resolve suggest an immune or metabolic response — celiac and HFI territory. Knowing when your symptoms hit and where they live in the abdomen is useful information to bring into the diagnostic conversation.

Treatment

The goal of Crohn's treatment is sustained remission — getting inflammation under control and keeping it there. Symptom control alone is not sufficient. Active inflammation causes progressive damage regardless of whether symptoms feel manageable on a given day.

5-ASA medications (mesalamine) — used in UC as a first-line treatment but evidence for benefit in Crohn's is limited. Not the primary tool for Crohn's management.

Corticosteroids — used for flare induction. Work fast. Not for long-term maintenance. Budesonide is preferred for ileocolonic Crohn's because of its localized action and lower systemic side effect profile compared to prednisone.

Immunomodulators — azathioprine, 6-mercaptopurine, methotrexate. Used for maintenance. Slow to reach full effect — weeks to months. Not rescue therapy.

Biologics are the current standard of care for moderate to severe Crohn's and for fistulizing disease. TNF inhibitors (infliximab, adalimumab), vedolizumab, ustekinumab, and risankizumab. These medications have changed the long-term disease course for many people with Crohn's. They are not light medications and they require monitoring. For fistulizing disease specifically, infliximab has the strongest evidence base.

Surgery manages complications — strictures, fistulas, abscesses, bowel obstruction — but does not cure the disease. Resection of a diseased segment can produce meaningful remission periods. Disease recurrence in remaining bowel is common. The goal of modern Crohn's management is to avoid or delay surgery through effective medical therapy while not hesitating when surgery is genuinely the right tool for a specific complication.

Nutritional support — exclusive enteral nutrition (liquid formula feeding) is an effective induction therapy in Crohn's, particularly in pediatric patients, and is underutilized in adult care. When significant malnutrition is present, nutritional rehabilitation is part of the treatment plan, not an afterthought.

What You Can Do Right Now

You cannot self-diagnose Crohn's disease. You can be better prepared for the diagnostic conversation and more persistent about getting the right workup.

If you are dealing with chronic right lower quadrant pain, unexplained weight loss, chronic diarrhea, or perianal disease that is not responding to standard treatment — inflammatory bowel disease belongs in the conversation. Push for a gastroenterology referral and push for the right imaging.

If you have had an appendectomy with a normal appendix result and GI symptoms continued afterward — bring that information explicitly into the gastroenterology conversation.

If you already have a Crohn's diagnosis:

  • Know where your disease is located. Location drives everything — treatment selection, monitoring, complication risk, nutritional concerns.

  • Know whether you are in remission or active disease. Symptoms do not always correlate with inflammatory activity. Active inflammation causes damage whether you feel it acutely or not.

  • If terminal ileum is involved, make sure B12 is being monitored.

  • Ask about fecal calprotectin for between-scope monitoring of disease activity.

  • Ask specifically for CT enterography or MR enterography — not standard abdominal CT — when small bowel imaging is ordered.

  • Ask that terminal ileum intubation and biopsy be performed during colonoscopy.

  • If fistula or perianal disease is present, make sure both gastroenterology and colorectal surgery are involved in your care.

A Note From Me

Crohn's honestly never made it very far onto my working board. The symptom picture didn't point there. I never had the right lower quadrant pain that puts Crohn's ileitis in the conversation. I never had the perianal disease. The presentation I was living with didn't match the failure pattern for this component, so I didn't chase it hard.

What I did research carefully — because my mom carries both Crohn's and UC diagnoses — is whether there was a family pattern worth paying attention to. My working theory on her situation is that her GI conditions are a downstream product of unmanaged celiac disease. She has never gone grain free. Celiac-driven chronic gut inflammation that never gets addressed has to go somewhere. Whether it gets labeled Crohn's or UC in that context is a question worth sitting with.

That's personal testimony and a mechanic's hypothesis. Not a medical claim.

What I can tell you is that my own GI picture was being driven by inputs — what I was eating and what my body couldn't process. When celiac and HFI were identified and managed, the GI chaos resolved in ways that autonomous inflammatory bowel disease simply does not respond to through diet. The damage wasn't structural. It was being driven by what I was putting in.

If your picture matches Crohn's — the skip lesions, the terminal ileum involvement, the right lower quadrant pain that doesn't move when you change what you eat — that's a real diagnosis that deserves aggressive targeted treatment. Push for the right imaging by name. Understand what surgery can and cannot do before that conversation gets forced on you in an emergency.
